Need a physical for the upcoming school year? The Mobile Health Unit will be on-site Thursday to help get your student cleared and ready to play!

Event Details:

• Date: Thursday, July 30

• Time: 10:00 AM – 3:00 PM

• Location: Alice Drive Middle School

• Cost: $20 (Cash or Card accepted)

Important Information:

• Forms Provided: You do not need to bring printed forms—the Mobile Health Unit has the correct physical forms available on-site.

• Paperwork: All required consent and authorization paperwork will be available to complete and sign when you arrive.
