IMMUNIZATIONS
Please be aware that the status of your child’s immunizations required for school attendance by SCDHEC is currently being audited district-wide. Students new to Sumter School District must complete SC Immunization Form DHEC 2740. A child cannot be admitted to any school or child development program without a valid SC Certificate of Immunization or a medical, religious, or special exemption. For details, contact the Health Department or your physician. According to state law, if your child is transferring from an out-of-state school, his/her health certificate information must be on a South Carolina form. Out-of-state certificates may be taken to a doctor or clinic in South Carolina to inquire what previous information can be transferred to the South Carolina form. Please note the 2024-2025 Immunization Requirements.
If your child’s immunization record is found to be missing or invalid, you will be notified by letter with a request to present a Valid SC Immunization Certificate within 30 days.

SC 5K-12 Immunization Info for Parents English
Por favor, tenga en cuenta que el estado de las vacunas requeridas para la asistencia escolar de su hijo/a según el SCDHEC se está auditando actualmente en todo el distrito. Los estudiantes nuevos en el Distrito Escolar de Sumter deben completar el Formulario de Inmunización de Carolina del Sur DHEC 2740. Un niño/a no puede ser admitido/a en ninguna escuela o programa de desarrollo infantil sin un Certificado de Inmunización válido de Carolina del Sur o una exención médica, religiosa o especial. Para más detalles, contacte al Departamento de Salud o a su médico. Según la ley estatal, si su hijo/a se está transfiriendo de una escuela fuera del estado, la información de su certificado de salud debe estar en un formulario de Carolina del Sur. Los certificados de fuera del estado pueden llevarse a un médico o clínica en Carolina del Sur para consultar qué información previa se puede transferir al formulario de Carolina del Sur. Por favor, tenga en cuenta los Requisitos de Inmunización para el período 2024-2025.
Si se encuentra que el registro de vacunación de su hijo/a está incompleto o inválido, recibirá una notificación por carta solicitando que presente un Certificado de Inmunización válido de Carolina del Sur dentro de los 30 días.
